PacifiCare Self-Directed Health Plan

PacifiCare Individual Self Directed Health Plan is a lower-cost, PPO-based plan that gives you access to a broad network of physicians and hospitals. The PacifiCare SignatureFreedom plan provides you an annual Self Directed Account (SDA) of $1,000 ($250 per quarter) for an individual or $2,000 ($500 per quarter) for a family which can go toward routine health care services like doctors' office visits, mammography screenings, breast and pelvic exams, and more.
- Immediate coverage through your Self Directed Account (SDA) for routine and preventive care, meaning these services will be paid in full up to the SDA dollar limit of your SDA account
- Services paid by the SDA are credited toward your annual deductible
- Any unused SDA funds carry over year after year for as long as you are enrolled on the plan, allowing you to "save" for future health care expenses
- Services not covered by the SDA, such as inpatient surgery or emergency room visits, are paid for like a traditional PPO, with a yearly deductible and a charge for coinsurance
- Access to a complete suite of online support tools that allow you to keep track of your SDA account, claim activity status, etc.
- With the SDA paying for certain services, you might not have any out-of-pocket expenses all year long
- Prescription Drug coverage, including a mail order option
